Sara Cleereman - RN, BSN, CFCN Community Health Nurse

 Professional & Personal Background

Sara has worked for the Community Health Department as a Community Health Nurse Generalist since 2013. She provides skilled nursing services, case management, health education, smoking cessation, medication management and discharge planning. She works individually with Tribal Members to plan, develop and implement patient centered care plans that promote self-management. Sara also coordinates the FCP In-Home Care Program, which ensures Tribal Members have the resources they need to live independently in their homes. Sara has previous experience working in hospice and long term care facilities, which have provided a foundation in the skills needed to develop lasting nurse-patient relationships.
