Community Health Assessment: Your Voice Matters!

The Forest County Potawatomi Community Health Department is conducting its second Community Health Assessment. The aim of this assessment is to identify the major needs and concerns of the Forest County Potawatomi Community, so the Community Health Department and others can better serve YOU. The assessment made its debut at the Winter’s End Pow Wow (March 18 – 19) and will continue to be implemented through the summer at community health events.

Two assessments are being provided to the community a youth assessment (ages 12 – 17) and an adult assessment (ages 18 +). These assessments provide the Forest County Potawatomi Community Health Department insight into the strengths of the Tribe, as well as areas that need more attention. YOUR VOICE MATTERS, and with your help and participation, we can gain a better understanding of the Tribe’s needs and improve the health and wellness of the community and future generations to come.
