Professional & Personal Background
Lisa has worked for the Community Health Department as a Nurse Case Manager with the In-Home program since December 2021. She offers education and case management to Forest County Potawatomi tribal elders and disabled adults. She provides office and home visits, needs assessment for in-home care services, nursing assessments, case management, home safety assessments, immunizations, medication management and support and education about various health and safety topics.
Prior to working for the Community Health Department, Lisa has worked in a variety of healthcare settings, including Veterans Health, Primary Care, Oncology, but mostly Long-Term Care Nursing facilities with over 25 years of nursing experience. She is passionate about providing quality care while remaining mindful of individual preferences and needs. In her free time, she enjoys spending time with her family and anything that has to do with the great outdoors.
