Elizabeth Reynolds - Social Worker

Elizabeth began working with the Forest County Potawatomi Health Department in November 2018. Elizabeth recently returned to the Wisconsin north woods, after living in Phoenix, Arizona, for 20 years, working for the State of Arizona as a Child Safety Specialist. As a social worker for the health department, Elizabeth serves as an advocate for Community Health, all residents at The Caring Place, and the elderly, assisting in helping people access services and education related to health care and safety.

Melanie Tatge

Melanie Tatge - Community Health Outreach Manager

Melanie began working with the Forest County Potawatomi Community Health Department Public Health Educator/Accreditation Coordinator in March of 2016. During her initial years with the Forest County Potawatomi Community Health Department, she gained an appreciated for the Forest County Potawatomi Community that led her to apply for the Community Health Outreach Manager position. She began her journey as Community Health Outreach Manager in November 2017. As Community Health Outreach Manager, Melanie oversees the everyday operations of the Forest County Potawatomi Community Health Department, Staff, Programs and Services.

Ann Chrisman

Anne Chrisman - RN, BSN, Diabetes Coordinator

Anne has worked for the Community Health Department as the Diabetes Coordinator since 2013. She provides skilled nursing services and case management to Tribal Members of all ages, but specializes in diabetes care. Anne provides office/home visits, nursing services, screenings, self-management education and support to individual who are at risk or diagnosed with diabetes. Prior to working for the Community Health Department, she worked as the lead nurse in Family Practice for 15 years. Anne also has hospital experience in cardiology, surgery and ICU.

Becky Meinert

Becky Meinert - LPN, Community Health Representative (CHR)

Becky has been working for the Forest County Potawatomi (FCP) Health Division since 2007. She has worked in the Community Health Department as a Community Health Representative (CHR) since 2010. Becky provides office/home visits, vaccinations, blood pressure screenings, blood glucose checks, medication deliveries and ensures that patient’s healthcare needs are being met. Prior to working in the Community Health Department she worked in the FCP Medical Department.

Cathy Chitko

Cathy Chitko - Assistant Diabetes Coordinator/Community Health Representative (CHR)

Cathy has worked for the Community Health Department as a Community Health Representative (CHR) since 1999. In 2005, she began her position as the Assistant Diabetes Coordinator. Cathy provides services to Tribal Members of all ages, but specializes in working with individuals who are at risk or diagnosed with diabetes. Cathy provides office/home visits, blood pressure screenings, blood glucose checks, medication deliveries and ensures that patient’s healthcare needs are being met.

Cathy worked as an Emergency Medical Technician-Basic for 18 years and also as the Goodman-Armstrong Creek Rescue Squad Member and Training Officer for 16 years.

Jodie Harris

Jodie Harris - RN, BSN, CLC, LCCE, Maternal Child Health Nurse

Jodie has worked for the Community Health Department as a Maternal Child Health Nurse since 2003. She took a one year hiatus in 2013 and returned in 2014 to a job she has always had a passion for. Jodie educates new and expecting mothers and fathers about the importance of healthy lifestyle choices and provides them with necessary skills to ensure their children have safe and healthy beginnings. She provides home and office visits, infant assessments, prenatal and postpartum assessments, childhood immunizations, breastfeeding support, childbirth classes and information about various health and safety topics.
