Tracie has worked for the Community Health Department as a Community Health Representative/Assistant Diabetes Program Coordinator since September 2023. She provides support to Forest County Potawatomi Community members living with diabetes. She provides diabetes case management, blood glucose level management, home/office visits, medication management, in-home care services, support and education about various health and safety topics.
